Dual M-Stent Procedure Combines Transpapillary and Percutaneous Techniques at Antwerp University Hospital

On December 19, 2024, a complex and successful biliary drainage procedure was performed using 2 M-stents in a "One-Step Side-by-Side" approach.

Procedure Highlights:
A plastic biliary stent (15 cm) was initially placed in the left hepatic duct to ensure drainage.
- The plastic stent was removed and replaced by a Visiglide 0.025" guidewire to the left hepatic duct.
- Another Visiglide 0.025" guidewire was inserted percutaneously through the pigtail drainage catheter, reaching the duodenum.
Innovative Technique Execution:
- Instead of a rendezvous approach, the decision was made to place two M-stents simultaneously:
- The first M-stent was advanced transpapillary into the left hepatic duct.
- The second M-stent was introduced percutaneously into the right hepatic duct, crossing into the duodenum.
- The left hepatic stent (12 cm x 8 mm) was fully deployed first, followed by precise deployment of the right hepatic stent (12 cm x 8 mm) to ensure optimal alignment and drainage.
